E-Statement Disclosure
eStatement(s) Election – You may elect to
receive your First Choice America Community Federal Credit Union account statement(s)
online, via First Choice America Online. This includes both your regular account statement and,
if applicable, your VISA Credit Card statement. In order to receive the current month’s statement(s)
electronically, you must make your election by 7:00 pm EST on the last business
day of the month. Saturdays,
Sundays, and federal holidays are not business days.
Elections made after 7:00 pm EST on the last business day of the month
will be effective the following month.
Revocation
of eStatement(s) Election
– You may at any time withdraw your
consent and revoke your agreement to receive electronic statements and
disclosures by changing the Preferences on your online account to
U.S. Mail. To be effective
for the current month, changes must be made by 7:00 pm EST on the last
business day of the month. Saturdays,
Sundays, and federal holidays are not business days.
Changes made after 7:00 pm EST on the last business day of the
month will be effective the following month.
Paper
Statements
– The Credit Union will discontinue mailing your
statement(s) in paper form the fourth month after you elect to receive
electronic statements. For
the first three months, you will receive both paper and electronic
statements. After the paper statements are discontinued, the Credit Union
will mail you a paper statement, at any time upon request, subject to the
Statement Photocopy Fee stated in the Credit Union’s Fee Schedule.
eStatement(s)
– eStatements will generally be available for viewing on the first
business day of the month or quarter. Saturdays, Sundays, and legal
holidays are not business days. An Alert will appear on the First Choice
America Online site to advise you that your statement is available for viewing.
The eStatements system will maintain copies of your past statements for up
to eighteen (18) months. Initially
you will be able to view your past statement(s) from March 31, 2003 and
forward. If your account was
opened prior to March 31, 2003, you will be able to view your past
statement(s) for an eighteen (18) month period beginning with the
September 30, 2004 statement.
VISA Credit Card Payment Form – You understand that by electing to receive your VISA account statement electronically, you will not receive a VISA Credit Card Payment form. You may transfer your monthly VISA Credit Card payment electronically via Steel Works Online, or you may remit your payment by check to First Choice Amercia Community Federal Credit Union at P. O. Box 762, Weirton, WV 26062, or you may make your payment in person at any of the Credit Union’s offices. Payments received more than 15 days after the stated payment due date will be subject to a late charge
The Briefer
– You understand that by electing to receive your account statement(s)
electronically, you will no longer receive a paper copy of the quarterly
newsletter. You may view and/or print a copy of the Briefer on the
Credit Union’s web site at www.firstchoiceamericacu.org
or you may obtain a paper copy at any First Choice America Community Federal Credit
Union office. Please review
the Briefer for the latest information about the events and happenings at
your Credit Union, as well as up-to-date information about the Credit
Union’s new products and services and any product or service changes or
regulatory changes.
Checking
Account Reconciliation Form/Statement Disclosure – A Checking
Account Reconciliation Form/Statement Disclosure is available on the
Credit Union’s website at www.firstchoiceamericacu.org
Please use this form to reconcile your checking account. You should
report any errors or irregularities to the Credit Union immediately.
